The problem
Healthcare organisations measure almost everything, yet leaders still struggle to answer the questions that matter. What is actually happening? Who owns it? What needs attention first? Can a serious failure hide behind good performance elsewhere?
PCM defines governance as the conversion of distributed performance information into accountable decisions, within the finite attention of the people who govern.
How PCM works
The compliance gate
In PCM's scoring function, S(u) = 2K(u) + A(u), K is the compliance indicator (1 if no critical failures, 0 if any exist) and A is the weighted aggregate of substitutable domains. Compliant units score in [2,3]; units with critical failures score in [0,1]. The gap between 1 and 2 is absolute. No amount of substitutable strength can bridge it.
Up the hierarchy, compliance is never averaged: a parent is compliant only if every unit beneath it is, K(P) = min K(Ci). One failing ward makes the hospital non-compliant, whatever its other results.
Under PCM, an aggregate must never be reported as a bare number: the pillar profile is always shown beside it. That is why the illustration shows A, K and S separately.
Six foundational commitments
Governed decisions must be traceable to objective evidence about performance. The framework rejects governance by assertion.
Leaders’ attention is finite, so governance outputs are reduced to what is relevant to decisions rather than everything that can be measured.
Every surfaced decision has an identifiable owner. Analytical systems can inform decisions, but they can never become the accountable decision-maker.
Every score, signal and decision can be reconstructed from its inputs: where the number came from, what evidence produced it, what rule was applied and who made the judgement.
PCM applies its own discipline to itself. Its claims must remain capable of being shown wrong, and it states the conditions under which it should be revised or abandoned.
Patient welfare is the terminal criterion. Efficiency, finance and other objectives serve it and can never become unlimited substitutes for it. The paper names this openly as a normative commitment.
